Learn how the Coming Home mobile app connects to the web platform/CCMS, and where that collaboration lives on both sides.

What is App and System Connectivity?

Coming Home's mobile app extends the Coming Home experience beyond the provider-facing web platform, giving clients a simple, secure way to stay connected with their care team between moments of care. Connectivity refers to the features that link what a care provider does in the CCMS to what a client sees and does in the mobile app, and vice versa.

This connective layer is built on three core features:

  • Assignments — goals, tasks, and challenges assigned by a provider, visible and trackable by the client
  • Chat — secure messaging between a client and their care provider
  • Journal — a private space for client reflection, not visible to providers

Who Can Access These Features

  1. Care Providers / Team Members
    • Can enable mobile app access for a client
    • Can assign goals, tasks, and challenges
    • Can message clients through the CCMS
    • Can message other team members within their organization
    • Receive notifications when a client sends a message or completes an assignment
    • Cannot view a client's private journal entries
  2. Clients
    • Can log into the mobile app using credentials created by their care provider
    • Can view and update progress on assigned goals, tasks, and challenges
    • Can message their care provider
    • Can use the private journal for personal reflection
    • Cannot self-register for app access

This access model keeps the client experience connected to the organization's care workflow while protecting client privacy.

 

What You Can Do With App Connectivity

The table below shows how each connective feature works on both sides of the platform.

1. Assignments

Providers assign goals, tasks, or challenges in the CCMS. Clients view and update their progress from the mobile app, keeping them connected to their care plan between sessions.

Includes:

  • Goals, tasks, and challenges assigned from the CCMS
  • Progress updates made by the client from their phone
  • Visibility for providers into completion status

Why it matters: Care shouldn't stop between sessions. Assignments give clients a way to keep working toward their goals, and give providers visibility without requiring an in-person check-in.

2. Chat

Clients can message their care provider directly from the app. Providers receive and respond to those messages from the CCMS.

Includes:

  • Secure, one-to-one messaging between client and provider
  • Message history tied to the client's record
  • Provider notifications when a new message arrives

Why it matters: This creates a connected communication loop, so clients have a way to reach their care team even outside scheduled sessions.

3. Journal

Clients can use the mobile app's private journal to reflect and track their thoughts over time.

Includes:

  • Free-text journal entries created by the client
  • Entries that are private to the client only

Why it matters: Not every reflection needs to be shared. The journal gives clients a safe, private space of their own, which supports trust in the platform.


Tips/Best Practices

  • Introduce app access at the right time: Enable mobile app access when it fits your program's stage for that client, rather than defaulting to enabling it as early as possible.
  • Assign with intent: Use assignments to reinforce what's discussed in sessions, not just to fill space.
  • Check notifications regularly: Staying on top of client messages and completed assignments helps you respond in a timely way.
  • Be clear about journal privacy: Let clients know their journal is private, so they feel safe using it.
