Identity Arc™ is a structured progress-tracking framework that enables service providers to assign and monitor Challenges, Goals, and Tasks (C, G, T) for each client. Located within the Client Toolkit, this feature provides a real-time overview of a client’s journey, helping teams support long-term transformation while maintaining accountability and measurable outcomes.
Identity Arc centralizes care planning and progress monitoring, ensuring that both providers and clients have a clear understanding of the steps required to achieve meaningful change.
Why it’s called Identity Arc
The term “Identity Arc” reflects the client’s personal journey of growth and transformation. Rather than focusing solely on services delivered, this framework emphasizes progress over time, guiding clients from their current circumstances toward long-term stability and well-being.
By structuring this journey into Challenges, Goals, and Tasks, providers can create actionable care plans that are easy to track and evaluate.
Key Components: Challenges, Goals, and Tasks (C, G, T)
Identity Arc is built on a three-tiered structure that organizes a client’s development:
1. Challenges (C) – Long-Term Objectives
Challenges represent the broader life areas a client aims to improve. They are strategic and outcome-oriented, often spanning an extended period.
Examples:
- Overcoming anxiety
- Achieving stable housing
- Securing long-term employment
2. Goals (G) – Short-Term Milestones

- Provider View (Client Toolkit):
Service providers can assign, monitor, and manage all Challenges, Goals, and Tasks from a centralized dashboard. This includes filtering assignments, tracking progress, and exporting data for reporting. - Client View (Mobile App):
All Challenges, Goals, and Tasks (C, G, T) assigned to a client are also visible within the client-facing mobile app. This empowers clients to:- Understand their care plan
- Track their own progress
- Stay engaged with assigned activities
- Take ownership of their personal journey

- Assignment Count: Total number of active items.
- Filtering Options: Toggle between Active, Archived, and All assignments.
- Search Functionality: Quickly locate specific Challenges, Goals, or Tasks.
- Add Assignment Button: Enables providers to create new assignments.

- 🟠 Not Completed – Active but unfinished
- 🟢 Completed – Successfully achieved

- Clients – Can view and engage with their assigned Challenges, Goals, and Tasks through the mobile app.
